Can someone explain the full process, it'd be great if someone who's involved with management (and making decisions) steps in and spills beans. I am sure many of us want to know about this.

I am a Store Manager at a Supercenter, I've been around for much longer than I wanted. Here is my two cents on this question... There's a standard practice expected of all Store Managers, which typically involves closing the store once a week and pulling an overnight shift monthly.
The Market Managers I've reported to have generally left it up to me to decide my schedule based on what the store needs, effectively giving me the reins. I usually plan my schedule 2-3 weeks ahead and inform my MHRM via text or email, and they'll let me know if adjustments are needed due to scheduling conflicts or other commitments.
My usual workday runs from 7am to 5pm, although I aim to do closing shifts twice a month (12pm-10pm) and an overnight shift every 2-3 months. I also step in to cover gaps in coverage when necessary (currently, I'm covering for an O/N ASM's PTO due to having only two on staff). My sleep schedule has been irregular since high school, so this variability doesn't affect me much.
Regarding time off, I typically submit them as calendar events to my MHRM and MM, and one of them signs off on it.
